Student registration

General Registration Requirements

• Copy of Birth Certificate
• Proof of  North Smithfield Residency – Residency Affidavit and one or more of the following*acceptable documents:  (1) sales or lease agreement; (2) tax bill; (3) recently dated utility bill
• Copy of Immunizations and Physical – all entering students must present evidence of immunization with month/day/year for the following immunizations:  Diphtheria, Tetanus, Pertussis, Oral Polio, HIB, Hepatitis, Measles, Mumps, and Rubella, HPV, Tuberculosis, Lead Screening, and Vision Screening.

• Transfer Students - All other academic records (grades, previous schedule, etc.) are needed to schedule appropriate classes.     Please complete "Transfer Students-Release of Records Form"

A Complete Registration Packet should contain the following 6 forms; Please download and complete the required forms.

1. Registration Form
2. Residency Affidavit
3. Home Language Survey
4. RI School Physical (or acceptable physician form) signed by Healthcare Provider
5. Health Information
6. Busing Form

All registration forms can be filled out electronically, printed and brought to the appropriate school for processing.
