Student registration

We are pleased to announce that we now have an online registration process. Click here to watch an informative video of the student online registration process. 

​Before you begin, please gather the following:

  • Household information -- address, phone numbers, proof of residency, and notarized Residency Form (form found below)
  • Parent information -- work and cell phone numbers, email addresses
  • Student information -- demographic and Birth Certificate
  • Health information -- RI School Physical Form (or acceptable physician form) signed by Healthcare Provider, Immunizations (all entering students must present evidence of immunization with month/day/year for the following immunizations:  Diphtheria, Tetanus, Pertussis, Oral Polio, HIB, Hepatitis, Measles, Mumps, and Rubella, HPV, Tuberculosis, Lead Screening),  Vision Screening and health conditions/medication information
  • Emergency Contact -- phone numbers.
  • Transportation Form (form found below)
  • Transfer Students - All other academic records (grades, previous schedule, etc.) are needed to schedule appropriate classes.     Please complete "Transfer Students-Release of Records Form"​

General Registration Requirements
  • Copy of Birth Certificate
  • Proof of  North Smithfield Residency – You must provide the appropriate Residency Affidavit and the necessary documents, detailed below:
    • PRIMARY Proof:  mortgage, purchase & sales agreement followed up with a closing disclosure or lease agreement.
    • SECONDARY Proof:  tax bill, utility bill dated within the last month.
  • Copy of Immunizations and Physical – all entering students must present evidence of immunization with month/day/year for the following immunizations:  Diphtheria, Tetanus, Pertussis, Oral Polio, HIB, Hepatitis, Measles, Mumps, and Rubella, HPV, Tuberculosis, Lead Screening, and Vision Screening.
  • Transfer Students - All other academic records (grades, previous schedule, etc.) are needed to schedule appropriate classes.  Please complete "Transfer Students-Release of Records Form"
